I've had my first such request today and am still developing a response plan that will work for all students should it come to that. Initially my thoughts are that they can obtain their grade/hours thus far and return to the university clinic until we see if those sites will let them back later. I've considered supplementing the university hours with simulated cases since we don't have a generous amount of clients to share at the university, but am honestly not 100% sure of my next steps. We have a plan to utilize work in Simucase with online consulting for debriefings for students to accrue some additional clock hours. These clock hours will be considered alternative clinical experiences (simulation), and can only account for a portion of the required contact time. One suggestion from our provost was to contact the accrediting body for direction on this. We are waiting on the instructions from our Dean regarding on-campus clinic closures, but we hope it will not come to that. I will be following your responses to see how others are proceeding with alternative placements. Dear Colleagues: Due to the COVID-19 pandemic, we are facing an unprecedented situation that is affecting the ability for clinical services to be provided on site and in person with clients and patients as required by the 2020 Audiology and Speech-Language Pathology Certification Standards. This situation extends to the ability for graduate student clinicians and Clinical Fellows to be physically present with clients/patients. The Council for Clinical Certification in Audiology and Speech-Language Pathology (CFCC) understands these concerns and, therefore, is providing an allowance for the use of telepractice and telesupervision for graduate student clinicians in CAA-accredited programs and for Clinical Fellows between March 16, 2020, and April 30, 2020, as described below: 1) Graduate student clinicians enrolled in CAA-accredited programs can engage in service delivery through telepractice when the clinical educator provides 100% direct supervision of the sessions in real time, either side-by-side with the student or with the student, the clinical educator, and the client/patient in different locations. This allowance is for both audiology and speech-language pathology programs. 2) Clinical Fellows may utilize telepractice provided an appropriately credentialed speech-language pathologist is available to assist in a similar manner to the supervision/assistance that is provided when services are delivered face-to-face. In addition, (1) the Clinical Fellow, the supervisor/Clinical Fellowship (CF) mentor, and the client/patient/student must all currently reside in the United States, and (2) the Clinical Fellow, the supervisor, and the CF mentor must be appropriately credentialed to provide services both in the state they reside and in the state(s) that they provide services. 3) Telesupervision may be used for direct, on-site, and in-person observations of Clinical Fellows by the CF mentor(s) for a segment of the CF experience that occurs between March 16, 2020, and April 30, 2020. As a reminder, a minimum of six hours of direct observations are required per segment (one-third of the CF experience) and up to six hours may be completed in one day. In all cases, clinical educators and CF mentors must meet the requirements as outlined in the 2020 Certification Standards. Additionally, all clinical educators, graduate students, Clinical Fellows, and CF mentors must ensure that telepractice and telesupervision are completed in accordance with HIPAA, FERPA, and prevailing local, state, and/or federal regulations. Simulation hours are being accepted by NC BOE-not new. NC BOE is accepting unlimited simulation hours but you still have to make sure ASHA guidelines are met for clinical simulation. So you have to go by ASHA guidelines for overall hours and if those are met and you still need to meet 20 hour distributions NC BOE will accept more hours of simulation to make the distributions. Ex. Student has 450 overall hours and still needs 2 hours to meet 20 hours of Child SP DX-even if they have acquired the limit of clinical simulation hours allowed by ASHA in their 450 NC will allow the additional 2 hours of clinical simulation to meet the NC requirement. That was how I understood it in my conversation with Denise at the BOE office. If you still have questions call and talk to her. You can?t change what ASHA requires-NC BOE is allowing some options to meet their requirements. Hope that helps.
