Please join us for the next free ACTS 2 Program Facebook Live Workshop, "Navigating the Transition from Home to Long-term Assisted Care for Adults with Dementia: What to Know and Ask," 7:00 PM-8:15 PM (Eastern Time) Thursday, August 17, 2023!



This workshop provides practical information about making the transition from home to long-term assisted care for loved ones with dementia. Caregivers often have questions about these facilities, want to learn more about them, and ask questions to those in the know. Here's your opportunity! Workshop presenters, Leslie Provis and Mary Beth Mann, leaders in the field of dementia and assisted care, will cover key topics, such as knowing when to make the transition from home to long-term assisted care, identifying options for assisted care, and determining the costs of such services. Felicia Reid, family caregiver, will share her personal experiences and lessons learned in moving her loved one with dementia to long-term assisted care. Tomeka Norton-Brown, ACTS2 Project Coordinator, will provide information about faith-based skills training and support for dementia caregivers and local, state, and national dementia care resources. Current and future caregivers, elder care professionals, and other interested community members are strongly encouraged to join the workshop.
