Hi Neil,
I wanted to touch base and let you know that I will be submitting to this opportunity. I have discussed it with David Lipten and we are awaiting the new application materials set to be released 1/29. I am currently putting the team together and anticipate collaboration with Rob Glueckauf from ISL among other FSU colleagues.
Thank you for sending this out - although the email trail noted that nursing had passed.....I never saw it through departmental communication.
Best,
Lucinda

Hello,

Not sure if this is something that fits anyone’s work that you know of. But, Nursing passed on it and Laurel Fulkerson thought it might work for someone. If so, please distribute, etc. Thanks.

Abstract

The goal of the Hillman Innovations in Care Program is to advance leading edge,nursing-driven models of care that will improve the health and health care of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and others.

We seek bold, evidence-based, patient- and family-centered approaches that challenge conventional strategies, improve health outcomes, lower costs, and show potential for national replicability.

The Rita & Alex Hillman Foundation specifically seeks proposals that address the healthcare needs of vulnerable populations in the following areas:
- Maternal and Child Health
- Care of the Older Adult
- Chronic Illness Management
