<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%">Workers’ Compensation reporting related to COVID-19 remains the same as the standard reporting protocol. Department representatives and supervisors should continue to report work-related injuries
or illness to AmeriSys at (800) 455-2079 and complete the standard <a href="https://hr.fsu.edu/pdf/forms/accinv.pdf">
<span style="color:blue">Accident Investigation Report</span></a> (AIR). Please document in the report if the claim is COVID-19 related. All Workers’ Compensation information, including forms, may be found on the Workers’ Compensation
