I hope that you are all doing well as the semester is winding down. If you are applying for, or if you are completing, an undergraduate or graduate certificate program offered in the College of Education, Health, and Human Sciences (CEHHS), please carefully read the email below. You may disregard this email, if you are: 1. Not applying for admission to a College of Education, Health, and Human Sciences (CEHHS) certificate program in the list below, or you are 2. Not submitting a certificate completion form (in fall 2023) as a student who is already admitted to one of the CEHHS certificate programs listed below** Certificate Program Admissions: Submitting a certificate admission application? Please see the specific certificate program information and website links listed below. For questions, please directly email the program-specific contact. Submit the online CEHHS Certificate Admission Applications early! December 15, 2023 is the last day of the semester in the fall 2023 semester. This also serves as the deadline for completed and department-approved CEHHS online Certificate Admission Forms to be received in the Registrar?s Office for fall 2023.** **In accordance with Florida State University policy, admission to College of Education, Health, and Human Sciences certificate programs requires submission of the online CEHHS Certificate Admissions Form to the appropriate department Certificate Program Coordinator and to the Registrar?s Office prior to the completion of two courses in the certificate program (grades posting for those courses). The University Registrar?s Office approval of the online certificate program application, prior to completion of two certificate program courses, is required. Important: Students are not admitted by the University into a certificate program by virtue of enrollment in certificate-related courses. An approved application is required. If you do not know if you have applied for and been approved for admission in a CEHHS certificate program, email the applicable ?Program Contact Email? in the College of Education, Health, and Human Sciences certificate programs list provided below. Certificate Program Completions: Submitting a certificate completion form? If you have been previously admitted by the University to a College of Education, Health, and Human Sciences certificate program** and you have completed all certificate program requirements, please see the respective certificate website links listed below for the applicable completion forms link and certificate contact information. December 15 is also the date that admitted certificate program students who are completing certificate in fall 2023 can begin to submit the online Certificate Completion Form(s) to the appropriate academic department/program for review. This is to ensure that all CEHHS certificate admission applications and certificate completion forms are received by the University Registrar?s Office in a timely manner and in compliance with Florida State University requirements for certificate programs. After all certificate coursework is completed, but no later than 2 weeks after grades post, the appropriate online CEHHS Certificate Completion Form must be filled out and submitted to the department Certificate Program Coordinator. Approved online completion forms will be forwarded to the Registrar?s Office for processing and posting to the official transcript. Certificate completion forms submitted by students who were not previously approved for admission to the certificate program will be denied. As previously mentioned in this email, approval of a certificate completion form requires previous approval for admission to the certificate.** Certificate Programs vs. Educator Preparation (Teacher Certification Track) Degrees: Are you a student in a BS/MS Pathway in the School of Teacher Education or the Educational Leadership/Administration MS or EDS degree programs? If so, do not submit a certificate completion form, unless you are also currently admitted in one of the College of Education, Health, and Human Sciences certificate programs. Certificate programs are NOT the same as Educator Preparation (teacher certification track) degree programs (BS, EDS, or MS), such as the Teacher Education Pathways. You do not need to complete a CEHHS Certificate Completion Form if you are enrolled in a State of Florida-approved initial certification Educator Preparation Program and applying for a teaching certificate with the Florida Department of Education after degree posting. * The Certificate Programs listed below are not teacher certification track academic degree programs earned in the School of Teacher Education. * The Certificate programs in the list below DO NOT lead to initial teacher certification in that subject area in the State of Florida. Jeong-Su Kim passed away on December 29 following a period of illness. He was surrounded by his family and friends, and is survived by his wife and daughters. I know I share the sentiments of our faculty, staff and students when I say we are all honored to have known Dr. J. Kim, who enriched the lives of many during his 16 years with the college. He will be missed immensely. Dr. J. Kim joined what was then the College of Health and Human Sciences as an assistant professor in 2006 and was an invaluable and highly respected leader, collaborator, researcher, instructor, mentor and friend. More importantly, his kindness, enthusiasm and passion for his work, our college, and programs undoubtedly encouraged and inspired countless students. Through his roles as graduate program director for the Department of Health, Nutrition, and Food Sciences, associate director of the Center for Advancing Exercise and Nutrition Research on Aging, and as a faculty affiliate of the Institute for Successful Longevity, the Pepper Institute, and the Institute of Sports Science & Medicine, Dr. J. Kim?s impact and contributions spanned the wider FSU community and beyond. The college is forever grateful for Dr. J. Kim?s warm spirit, generosity, and genuine kindheartedness. Gifts in honor of Dr. Kim may be made through the FSU Foundation and will fund the creation of a Dr. Jeong-Su Kim Graduate Scholarship and Research Fund in the department of Health, Nutrition, and Food Sciences. Donations can also be made directly to the family via Zelle (850-339-0597) or Venmo (@youngim420). His family is planning a memorial service on January 6, 2024, at 11 AM at Tallahassee Korean Baptist Church, located at 4225 Crawfordville Road, Tallahassee, FL, 32305. The service will also be streamed on YouTube here.
